• Whether you'd like this sent to you or straight to the giftee (if you, we will leave the name blank)

• Either your full name and address, or the name and address of the giftee for us to post the gift card to

Gift cards are non-refundable
Gift cards are valid for 12 months from date of purchase
Gift cards can be used towards any treatment offered by The Skin Clinic.
